  • Overview and Responsibilities

    The individual in this position coordinates and facilitates clinical and business functions in support of oncology referral services and referral development department to include: processing patient referrals accurately and efficiently through data entry and with established efficiency and protocol guidelines; generation of reports, communication and follow up with physican office staff and patients, problem resolution and service recovery. Outreach to referring providers/clinics to promote referral services. Prepare new patient appointments with appropriate records, orders, and documentation.

     

    Responsibilities

    • Collaborates in overseeing the practice's referral coordination initiatives.
    • Assist with and facilitate the transition of care from referring provider/facility to receiving provider care.
    • Maintains strong working relationships with internal and external customers by providing service assistance and open communication.
    • Identifies and implements cost-effective problem solving, process improvement, risk assessment and the reduction of waste.
    • Perform other duties as assigned.

    Qualifications

    DescriptionMinimum RequiredPreferred / Desired
    Experience  
     Two years of nursing experience.Preferably within an oncology and/or clinic environment.
    Education  
     licensed LPNBSN
    Training  
     Excellent interpersonal skills with the ability to effectively communicate, both verbally anbd written, with patients, physicans and office staff to provide optimal levels of patient care. Skill and proficiency in applying the highly technical principles, concepts and techniques which are central to referral coordination and clinical protocol and guidelines. 
    Special Skills  
     Strong analytical and data management skills, comfortable seeking direction and assistance from appropriate resources, ability to communicate effectively verbally and written, proficient ability to use computer and electronic medical record 
    Licensure  
    LPN or RNLPNRN
