|Job Summary:||The Faith Community Nurse (FCN) will be an integral member of the multidisciplinary outreach team. In conjunction with assigned team members and the faith community, the FCN has responsibilities to provide direct and indirect faith community nursing care for individuals/patients and groups in assigned faith communities (e.g. congregations, faith-based community groups) or community organizations by acting as a resource, educator, and facilitator. FCN will provide health promotion and disease prevention services with an emphasis on spiritual care integrating physical and emotional aspects. FCN works under the direct and indirect supervision of Case Management and Pastoral Services but is independent in the application of nursing knowledge and skills. Manages community and clinical situations for which he/she is responsible. FCN behaves in a professional manner and consistently demonstrates and promotes the values of respect, honesty, care and dignity for the patient and all members of the healthcare team. The FCN is committed to the constant pursuit of excellence in improving the health status of the community. through our mission of Healing, Teaching and Preaching. FCN Performs other duites as assigned.|
