Overview and Responsibilities

Duties of the position include, but is not limited to the following:

  • Coordinates and provides care utilizing the critical thinking framework known as the nursing process. The nursing process forms the foundation of the nurse's decision making to help partner with patients/families to attain, maintain and restore health whenever possible.
  • Blends caring, compassion, knowledge and integrity to provide safe quality care that preserves patient autonomy, dignity and rights. Integrates practice with a service-first focus keeping within the Baptist Mission of the three fold ministry of Christ through healing, teaching and preaching.
  • Performs other duties as assigned.

 

 

Manages the referral to admission process by assessing patient and family status based on admission criteria to ensure the placement of all patients in the appropriate level of care. Develops and communicates the preliminary plan of care to all involved parties. Assumes ownership of referral development, marketing activities, and the collection of all statistical information related to these functions to ensure consistent patient census. Subject to overtime and callback as required. Reports to the Case Management-Social Work Manager. Performs other duties as assigned.

Qualifications

  • Experience Minimum Required: RN with 3 years acute care experience.
  • Experience Preferred/Desired: RN with 5 years acute care experience.
  • Minimum  Special Skills Description: Excellent communication skills. Advanced computer literacy skills.
  • Special Skills Minimum Required: Excellent communication skills. Advanced computer literacy skills.
  • Special Skills Desired/Preferred: Case management or marketing skills preferred.
  • Licensure Minimum: RN
